My personal pet peeve is seeing people (particularily previous sysadmins of the Computing society here...) use LVM to stripe together 2x physical disks as 1 partition, without layering this on top of any sort of RAID striping (hardware or mdadm). The state of the society's storage server at the beginning of the year made me almost physically sick; 4x physical disks, split into 2x LVMs (not on top of any RAID) each with a single partition spanning 2x disks, with a cron job that dd'ed one LVM to the other every night at midnight.
